and meaningful patient outcomes. You’ll use your clinical expertise
to manage resources, coordinate patient care from admission through
post-discharge, and guide interdisciplinary plan-of-care
decisions.Key Responsibilities: - Collaborate with the
interdisciplinary team to develop and adjust individualized care
plans. - Coordinate discharge planning, ensuring timely transitions
and appropriate follow-up care. - Facilitate weekly treatment
conferences and family meetings as needed. - Monitor patient care
metrics, compliance, and service appropriateness. - Educate
patients and families on the case management process and post-acute
needs. - Review case reports and utilization trends to drive
quality and efficiency. - Ensure accuracy of discharge and
payor-related information in patient records. - Participate in
on-call case management schedule as needed. Qualifications: - Must
be licensed in a qualifying discipline (RN, LCSW, OT, PT, ST,
Rehabilitation Counseling, etc.). - Associate’s Degree in Nursing
required (BSN preferred) OR - Bachelor’s Degree required for other
disciplines (graduate degree preferred). - Must be eligible for
CCM® or ACM™ certification upon hire; certification required within
2 years. - At least 2 years of experience in rehabilitation or
acute care preferred. - Strong knowledge of care coordination,
insurance requirements, and interdisciplinary treatment planning.

Candidate Profile: - Licensed RN or MSW with hospital or rehab
discharge planning experience - Skilled in interdisciplinary
collaboration and family communication - Detail-oriented with
strong documentation and compliance practices - Motivated to
