Nurse Practitioner Long-Term Care New Hampshire

We are seeking a Nurse Practitioner to join a facility-based medical team providing comprehensive primary and preventive care to patients residing in skilled nursing and long-term care facilities. This role offers the opportunity to make a meaningful impact on the health and well-being of the elderly population while working collaboratively within an interdisciplinary care team.

Responsibilities

• Provide primary and preventive care to long-term care residents

• Manage residents with acute changes in condition

• Perform advanced care planning and chronic care management

• Prescribe medications, including controlled substances, in accordance with NH law

• Order, review, and follow up on diagnostic testing and treatments

• Communicate regularly with nursing facility staff regarding patient condition and care plans

• Collaborate with physicians and interdisciplinary care teams

Schedule

• Monday Friday, 8:00 AM 5:00 PM EST

• 4 days per week (schedule coordinated with manager)

Qualifications

• Current, unrestricted Nurse Practitioner license in New Hampshire

• Board Certified NP

• Active DEA license

• Background in adult, family, or geriatric medicine

• Experience in a long-term care or skilled nursing setting preferred

• Proficiency with EMR systems

• Strong clinical judgment managing complex, chronic conditions
