Job Description:
Clinical Practice and OutcomesWorks collaboratively as part of a
team.Participates in NP mentoring. ImpactShares specialty knowledge
with staff.Applies principles of EBP and QI into patient care.
LeadershipAwareness of organizational mission and
goals.Committee/professional organization membership. ADDENDUM with
Psychiatric certificationMay assess patients' medical, behavioral,
and psychological needs and conditions, including, but not limited
to diagnostic assessments.May prescribe and monitor psychotropic
medications in collaboration with psychiatric teamMay provide
psycho-education, counseling, health promotion, and preventive
services.May communicate as appropriate with patients' relatives,
collateral contacts, and fellow professionals in the community to
develop and implement treatment plans.May provide physical health
screening and primary health care monitoringMay provide clinical
consultation to peers and other health care professionals.
